Staphylectomy using an ultrasonic scalpel (vetsonix®) in a dog: case report

Abstract

Introduction: Brachycephalic Obstructive Airway Syndrome is a well-known and very common condition in dog breeds with shortened craniofacial conformation, such as the Shih Tzu, characterized by alterations that compromise respiration. One of the main manifestations of this syndrome is the presence of excessive palatal tissue that extends beyond the epiglottis, causing airway obstruction, whose surgical correction can be performed through staphylectomy using a conventional scalpel, surgical scissors, ultrasonic scalpel, or various types of lasers. Objective: To report the case of a female dog submitted to the staphylectomy surgical technique using an ultrasonic scalpel. Case Report: A 9-year-old, 6kg female Shih Tzu with a history compatible with upper airway obstructive syndrome was treated at the Veterinary Teaching Polyclinic of the University Center of Valença. On physical examination, stenotic nostrils, respiratory effort, stertor, and an elongated soft palate were observed. Cervical and thoracic radiographs showed mild tracheal hypoplasia and no bronchopathy. Staphylectomy with an ultrasonic scalpel was indicated, an instrument that allows precise cuts, less bleeding and inflammation, resulting in a faster and more comfortable postoperative recovery. Postoperatively, analgesia, anti-inflammatory medication, and a soft diet were instituted, with only oropharyngeal edema, which was controlled with dexamethasone. On the fifteenth day, the patient presented silent breathing and significant improvement. Conclusion: Staphylectomy with an ultrasonic scalpel proved effective in improving the patient's respiration, providing rapid recovery and a better quality of life. This case highlights the importance of early diagnosis and reinforces the ultrasonic scalpel as a advantageous technique in veterinary surgical practice.
